Before the visit

Prepare the information your clinician may need.

Appointment length, frequency, forms, and preparation can vary by service. The team will tell you what applies when scheduling.

During and after care

Treatment planning should be understandable.

The clinician reviews the concern, relevant history, medicines, safety, functioning, and goals. Recommended next steps may include follow-up, testing, therapy, medication, recovery care, care coordination, or an outside referral.
