Use a valid authorization naming the recipient, purpose, information, and applicable dates.

Protecting confidentiality
Some information may have additional protections.
Identity and authority are verified before release. Psychotherapy notes and certain substance-use records may have additional confidentiality requirements. Applicable fees and response timeframes follow current law and Icare policy.
